FEE ITEMIZATION FOR DPC MEMBERSHIP
- 0-18 years of age $35 per month*
- 0-18 years of age $70 per month**
- 18-65 years of age $75 per month
- 65+ years of age $80 per month
- Family rate (up to 4) $150 per month
Enrollment Fee $50 (one-time fee)
Re-enrollment Fee $100
*with the enrollment of at least one adult member
**without a fully enrolled adult
Flow nor its Practitioners, participate in any health insurance or HMO plans or panels and have opted out of Medicare.

Is it after hours and your primary care is closed? Would you like to avoid the emergency department or urgent care and extended wait times? Not an established client at our practice, but have an urgent need?
No insurance or underinsured?
Flow is here to care for your needs! Sore throat, ear pain, minor injuries, simple lacerations, migraines, toothaches, or other acute conditions.
Telemedicine and office evaluation as indicated to meet your urgent care needs. Appointment required. Visits starting at $75.
How do I decide if an acute office visit is the right fit for my needs?
Knowing where to turn for a sudden health problem isn’t always easy. Some of the conditions that should prompt a visit to the nearest emergency room include:
· Chest pain
· Shortness of breath
· Loss of consciousness
· Uncontrolled bleeding
· Broken bones that puncture the skin
· Asthma attack
· Thoughts of suicide or self-harm
